Forms & Payment

Forms & Documents

After setting up your first appointment, Comfort will email you forms to fill out. Additionally, please provide copies of the following documents to the SLP at your child's first session:

  • Current IEP

  • Speech and language evaluations

  • Hearing tests

  • Recent medical physical, and/or relevant medical evaluations

  • Goals that are currently/were previously targeted in therapy

    • To include physical therapy, behavior intervention, occupational therapy, and/or other speech services

Insurance

At this time Comfort accepts the following insurances: 

  • Medicaid

Comfort provides super-bills to families that can be submitted to insurance companies for reimbursement.

Payment

Payment is due at the date of service. We currently accept the following forms of payment: 

  • Cash

  • Credit Card

  • HSA Card
